- Be US citizen or permanent resident
- Have a diagnosis of cancer
- Not have started cancer treatment
- Not have had recent chemotherapy treatment within prior six months
- Have oncologist and reproductive endocrinologist determine that fertility preservation treatment is medically appropriate
